Employee Eligibility
Employees are eligible to participate in the insurance plans after a waiting period of 60 days, if they work a minimum of 30 hours per week. Coverage will become effective for medical insurance after the 60 day waiting period and effective the first of the month following a 60 day waiting period for the dental, vision and supplemental insurances. Employees who work less than 30 hours per week are NOT eligible for benefits.

Tax Form Requests
Under new guidelines of the Paperwork Burden Reduction Act (PBRA) and the Employer Reporting Improvement Act (ERIA) signed into law on December 23, 2024, all 1095-C forms will only be distributed upon request. Please allow up to 30 days to receive the 1095-C form from the date the request has been submitted.
