Employees are eligible to participate in the insurance plans after a waiting period of 90 days, if they work a minimum of 30 hours per week. Coverage will become effective for medical insurance after the 90 day waiting period and effective the first of the month following a 90 day waiting period for the dental, vision and supplemental insurances. Employees who work less than 30 hours per week are NOT eligible for benefits.
Florida Blue – 4 Plans available
Medical insurance currently includes a menu of four plan options through Blue Cross Blue Shield, with F&H Contractors providing 100% of the cost of the employee-only BCBS HMO 47 plan. Spouse, dependent children, and family coverage is also available through all four plans, with the employee paying a pre-tax premium—after the company’s contribution—through weekly payroll deductions.
Basic Term Life Insurance
Company paid Basic Term Life Insurance for eligible employees only if a medical plan has been selected.
Term Life insurance benefit for active employees is up to $10,000.
Company sponsored dental plan through Humana to eligible employees. Employee is responsible for
premium with a payroll deduction.
Company sponsored supplemental coverage currently though Colonial Life and Accident Insurance Company. Employee is responsible for premium with a payroll deduction. All plans are portable at the same discounted group rate and guaranteed renewable for life. Available Supplemental Insurance Plans include Accident Insurance, Hospital Confinement, Specified Disease and Short Term Disability.
Eligibility: 90 days of service, must be 18 years of age or older, and work 1,000+ hours
Excluded: Part time employees with less than 1,000 hours annually.

Vacation and Paid Holidays
Vacation includes 1 week paid annually after 1 year of service, 2 weeks paid annually after 5 years of service and 3 weeks paid annually after 10 years of service. Additionally, full time employees receive 6 paid holidays annually.
